it depends on the current relationship. if you guys are really close now and have talked about possibly getting back together, then just do it. surprise her with flowers or something and tell her that you want to be together again. if you guys are on ok terms and you're not so sure how she feels, i'd say start planning more casual activities with her (for example, lunch dates) and as you get closer, start talking more and more about the possibility of being a couple again. at this point, if she seems into you then keep going out together and talking until you think you're both ready. if she seems to reject the idea of getting back together, either work at it slowly or just move on.
